Key Responsibilities

  • •Evaluate member needs via phone and in-home visits and recommend or facilitate individualized care plans/service plans.
  • •Develop ongoing care plans for members with high acuity, identifying providers, specialists, and community resources.
  • •Coordinate between members/family/caregivers, community resources, and the care provider team to ensure services are accessible.
  • •Monitor care plans/service plans and member outcomes, including treatment side effects, complications, and clinical symptoms.
  • •Collect, document, and maintain member information and care management activities to meet state, federal, and third-party payer compliance requirements.

Key Requirements
