  • •Serve as the attending physician for patients in each assigned SNF and oversee clinical supervision with NPs/PAs.
  • •Provide high-quality, patient-centered primary care, diagnosing and treating acute and chronic conditions.
  • •Participate in Theoria’s Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.
  • •Document efficiently using an EHR, utilize voice dictation and AI Scribe for documentation, and access UpToDate for clinical guidance.
