Key Responsibilities

  • •Support care management activities to ensure services are delivered by providers and partners and continuity of care is achieved.
  • •Perform telephonic member outreach or home visits to engage members and discuss care/service plans, next steps, and ongoing education.
  • •Coordinate care activities based on the care plan/service plan and work with healthcare/community providers, partners, and members/caregivers to accommodate changes.
  • •Handle member and provider inquiries, requests, and concerns related to the care plan/service plan.
  • •Document and maintain member records per state and regulatory requirements and make referrals to Care Manager, community organizations, and Disease Manager as needed.
