•Build referral partnerships: Develop relationships with physicians, care coordinators, social workers, assisted living facilities, and senior-focused organizations; educate providers on Sailor Health’s services, patient eligibility, and referral workflows.
•Own territory growth: Expand referral volume by developing new partnerships, managing high-priority accounts, and driving ongoing outreach in the local healthcare ecosystem.
•Operate in the field: Spend 4–5 days per week meeting partners in person across the territory and attend community events and provider meetings.
•Collaborate cross-functionally: Partner with clinical, operations, and patient experience teams to ensure a seamless referral and onboarding experience; share market feedback to improve provider experience and growth strategy.
•Communicate value with versatility: Present to groups of providers and perform 1-on-1 discussions with solo PCPs, balancing strategic territory planning with detailed follow-up.

Pay and Benefits

Salary: USD 70,000 - 95,000 annually
