Care Coordinator

Coordinate inpatient care for members in New Mexico through virtual and face-to-face interactions as required. Conduct in-depth health risk and needs assessments, develop and communicate individualized care plans, and monitor outcomes to ensure cost-effective, high-quality care. Serve as a point of contact during transitions, advocate for care needs by identifying gaps, educate providers and families, and generate care coordination reports while collaborating with interdisciplinary teams.
