Community Paramedic PRN

Deliver scheduled, patient-centered care as a Community Paramedic outside the traditional 911 response model. Conduct in-home assessments and follow-ups under physician-directed protocols for chronic and post-acute patients, document care in electronic medical records, and coordinate with the healthcare team to connect patients to appropriate resources. Partner with physicians to improve outcomes and reduce hospital readmissions while working in the Lansdale community on a per-diem basis.
