Bilingual RN Care Manager (Remote Flexible - Spanish Speaking)

Provide whole-person care management for individuals with complex medical needs, Serious Mental Illness, Substance Use Disorder, and/or homelessness. You will manage a caseload, develop Shared Care Plans, educate patients to improve health literacy, coordinate medication and prior authorizations, and track required assessments and screenings. Collaborate with a multidisciplinary care team through case reviews and triage escalations, while working primarily remotely with PST working hours.
