Key Responsibilities

  • •Conduct in-home assessments for Physical Disability & Aging Waiver members and evaluate needs and available resources.
  • •Assist with developing, coordinating, and monitoring holistic long-term care service care plans/service plans.
  • •Identify and coordinate needed providers, specialists, and community resources for member needs.
  • •Educate members and caregivers on services, benefits, referrals, procedures, and healthcare provider instructions.
  • •Maintain compliant documentation of member information and care management activities; perform on-site visits as appropriate and provide feedback to leadership.

Key Requirements

  • •Bachelor's degree and 1 year of related experience.
  • •For Illinois Plan Only: must meet one of the following—RN licensed in Illinois, or a Bachelor’s/Master’s degree prepared in a human services related field, or LPN with 1 year of experience conducting comprehensive assessments and providing formal service for the elderly.
  • •Conduct in-home assessments to evaluate member needs and available resources.
  • •Develop, coordinate, and monitor long-term care plans/service plans and ensure services are accessible.
  • •Collect, document, and maintain member information to ensure compliance with state, federal, and payer regulations.
