•Implement and support wellness visits, transitions of care, active management, and ensure practices meet goals in Practice Transformation Plans.
•Train office staff and physicians/providers on processes and technologies in coordination with the Value Based Care Curriculum.
•Continuously assess practice and staff training needs and adapt programs accordingly.
•Develop and nurture relationships with physicians, providers, and healthcare system executives to support and improve financial and quality performance.
•Listen to practices’ concerns and relay information to managers/leads to identify opportunities for improvement.

Key Requirements

•Must be a resident of Tennessee.
•Bachelor's degree or equivalent; 4-6 years of relevant medical practice management experience may be accepted in lieu of degree.
•3 years of experience with Electronic Health Records (EHR) for clinical/practice management processes
•Excellent oral and written communication, organizational and project management skills, and ability to build relationships with providers and executives
•Willingness to travel to practices up to 25% across Tennessee (approximately 1 week per month)
