Community Resource Coordinator II

Support community connection activities by connecting members to known community and care resources that help their care management journey. Coordinate community outreach, respond to community and care resource inquiries, and maintain documentation of resources to meet health plan standards. Conduct non-clinical health assessments and non-medical assessments (e.g., home safety, transportation, employment) and perform telephonic and/or in-person outreach, with support for the clinical team.
