Key Responsibilities

  • •Identify, outreach, engage, and enroll eligible individuals into the program virtually (phone, text, email) and in-person at clinic and partner locations.
  • •Listen openly to eligible members, meet them where they are, and support enrollment through multiple outreach attempts.
  • •Educate partners about Pair Team’s services and link eligible members to the Enhanced Care Management program.
  • •Explain Enhanced Care Management services and benefits, building trust with members who may be hesitant or distrustful.
  • •Manage the end-to-end enrollment funnel, including tracking funnel stages, delays/blockers, and meeting enrollment metrics.

Key Requirements

  • •Physically located in the United States and able to work a virtual/remote setup with a quiet, HIPAA-compliant internet-connected space.
  • •1+ year of experience working in a metric or quota-driven environment.
  • •Bilingual in English and Spanish with excellent communication skills (reading, writing, speaking, listening).
  • •Strong technical skills and comfort using multiple technology platforms for outreach and enrollment.
  • •Valid driver’s license and auto liability insurance.
