Key Responsibilities

•Function as the primary provider in the field, delivering both pre-scheduled visits and acute-response care directly in patients’ homes.
•Perform advanced nursing assessments, interventions, and follow-up care under physician-directed protocols.
•Provide care to stabilize patients, prevent unnecessary hospital readmissions, and support recovery.
•Collaborate with physicians, advanced providers, and EMS professionals to ensure seamless care transitions.
•Document care in electronic medical records and coordinate with the broader healthcare team.

Pay and Benefits

Salary: USD 40 hourly

Key Requirements

•Active Pennsylvania RN license (BSN preferred).
•Minimum 2 years of clinical experience (emergency/critical care/acute care/EMS preferred).
•Strong clinical judgment and communication skills; ability to work independently in dynamic field environments.
•BSN preferred; nursing degree and licensure required for practice.
•Interest in expanding into advanced EMS roles and home-based care settings.
