LTSS Service Care Coordinator

Coordinate holistic care management for long-term support services (LTSS) members, including assessing needs, developing and monitoring service care plans, and identifying providers and community resources. This hybrid-remote role includes 50% local home visits focused on Physically Disabled/Elderly waiver services, with ongoing education for members and caregivers. You’ll document and maintain care management records to support compliance and help improve quality and cost-effective outcomes.
