Key Responsibilities

  • •Provide high-quality, patient-facing care in members’ homes across Georgia.
  • •Manage a panel of medically complex Medicare Advantage members and take accountability for clinical outcomes.
  • •Conduct comprehensive in-home assessments across medical, functional, behavioral, and social needs.
  • •Lead post-discharge and transitional care visits to support safe recovery and reduce avoidable readmissions.
  • •Use Clover Assistant and clinical technology to surface risks, guide decisions, document accurately, and coordinate care with the interdisciplinary team.

Key Requirements

  • •MD or DO degree.
  • •Board certification in Internal Medicine, Family Medicine, Emergency Medicine, or a related primary care field.
