Key Responsibilities

  • •Review case load and prepare technology and tools to service patients.
  • •Navigate your schedule and coordinate with in-home clinical team members (Sprinters) visiting patients.
  • •Conduct wellness exams including health risk assessments, medication reviews, and cognitive screenings with patient education.
  • •Collaborate with Sprinters by evaluating vital signs and arranging blood draws and related tasks.
  • •Provide comprehensive care coordination and management including preventive interventions, medication management, referrals, community resources, and documentation.

Pay and Benefits

Salary: USD 120,000 - 130,000 annually

Key Requirements

  • •Board Certified as a Family Nurse Practitioner
  • •Active Family Nurse Practitioner License in New York
