Care Navigator

Develop and coordinate member-focused care management activities by assessing needs, barriers, and available resources. Create and update personalized care and service plans, connect members and caregivers to providers and community supports, and educate them on benefits and care options. Monitor progress, document and maintain member information to meet regulatory requirements, and provide guidance to care managers and healthcare teams as needed.
