Key Responsibilities

  • •Assess health and social service needs and promote well-being for community members through phone-based peer coaching.
  • •Follow provided processes and policies for engagement, documentation, tracking progress, and follow-through.
  • •Maintain appropriate conversation topics and build rapport to create a positive, welcoming experience.
  • •Use community resources, social services, and partnerships to meet members’ needs and relay questions/concerns to staff.
  • •Participate in onboarding, ongoing training, and orientation, and take part in local events when required.

Key Requirements

  • •Must be located in Sacramento County, California.
  • •Hold a valid certificate of completion pathway curriculum for CHW skills in communication, service coordination, assessment, outreach, evaluation, and public health principles/SDOH.
  • •Or have at least 2,000 hours working as a CHW in paid or volunteer roles within the previous three years, with skills and practical training in the listed areas.
  • •Reliable transportation and a flexible schedule for face-to-face outreach.
  • •Excellent written and verbal communication skills, plus strong computer skills to use web-based systems and maintain confidentiality under HIPAA guidelines.
