Key Responsibilities

  • •Navigate daily schedules and partner with in-home clinical team members for patient visits
  • •Conduct thorough wellness exams, including health risk assessments, medication reviews, and cognitive screenings
  • •Collect relevant patient information and coordinate proactive care using educational guidance
  • •Provide care coordination and management, including preventive interventions, medication management, specialist referrals, community resources, and documentation

Key Requirements

  • •Board Certified as a Family Nurse Practitioner
  • •Active Family Nurse Practitioner license in Louisiana
  • •Consistently demonstrate professionalism, integrity, accountability, confidentiality, care, and compassion
  • •Technologically savvy and comfortable using laptops or mobile devices for charting and HIPAA secure messaging apps for care coordination
  • •Pass a national background check and have a valid clinical license search
