•Collect relevant patient information and coordinate proactive care using educational guidance
•Provide care coordination and management, including preventive interventions, medication management, specialist referrals, community resources, and documentation

Key Requirements

•Board Certified as a Family Nurse Practitioner
•Active Family Nurse Practitioner license in Louisiana
•Consistently demonstrate professionalism, integrity, accountability, confidentiality, care, and compassion
•Technologically savvy and comfortable using laptops or mobile devices for charting and HIPAA secure messaging apps for care coordination
•Pass a national background check and have a valid clinical license search
