Key Responsibilities

  • •Begin each day by reviewing your caseload and preparing technology to support patient care
  • •Conduct proactive virtual wellness exams including health risk assessments, medication reviews, and cognitive screenings
  • •Connect with patients to collect relevant information and deliver personalized education about health and well-being
  • •Collaborate with the in-home clinical team to evaluate vital signs, arrange blood draws, and complete patient-specific tasks
  • •Provide comprehensive care coordination including preventive interventions, medication management, referrals, community resources, and documentation

Key Requirements

  • •Board Certified as a Family Nurse Practitioner
  • •Active Family Nurse Practitioner license in any of the following states: KY, MI, OH, IL
  • •Pass national background check and have a valid clinical license search
  • •Technologically savvy and comfortable using tools such as laptops or mobile devices for charting and HIPAA secure messaging apps
  • •Strong written and verbal communication skills with consistently professional, accountable, and confidential care
