Develop and coordinate care management activities based on member needs to improve access to high-quality, cost-effective healthcare. Assess barriers to care, build personalized care/service plans, and connect members and caregivers to healthcare providers and community resources such as housing, employment, and education. Provide psychosocial support, monitor progress, document care activities for compliance, and conduct education and coordination to ensure timely services.

•Develops, assesses, and coordinates care management activities based on member needs to improve healthcare outcomes.
•Creates and maintains personalized care/service plans and identifies required providers, specialists, and community resources.
•Provides psychosocial and resource support to members/caregivers to access services such as employment, education, housing, and food.
•Coordinates between members/families/caregivers and care provider teams to ensure accessible services in a timely manner.
•Collects, documents, and maintains member information and care management activities to ensure compliance with state, federal, and payer regulators.

Key Requirements

•Bachelor’s degree with 2–4 years of related experience.
•Graduate from an accredited school of nursing if holding clinical licensure.
•Equivalent experience demonstrating applicable knowledge, duties, scope, and skill at this level.
•Current state’s clinical license preferred.
•Ability to evaluate member needs, barriers to care, and available resources to recommend appropriate plans.
