Key Responsibilities

  • •Serve as the attending physician for patients in each assigned SNF.
  • •Collaborate with NPs/PAs to manage daily patient care and oversee clinical supervision.
  • •Provide high-quality, patient-centered primary care, including diagnosing and treating acute and chronic conditions.
  • •Participate in Theoria’s Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.
  • •Utilize an EHR and AI tools to document care efficiently and ensure high-quality clinical guidance.

Key Requirements

  • •MD/DO with active U.S. medical license and board-certified or board-eligible in an appropriate specialty (new graduates who are board-eligible are encouraged to apply).
  • •Specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
  • •Significant clinical experience in primary care and post-acute settings preferred.
  • •Ability to supervise and collaborate with NPs/PAs and other clinical staff.
  • •Willingness to participate in chronic care management, transitional care, remote patient monitoring, and quality initiatives.
