Care Coordinator II

Coordinate member care by conducting member outreach via phone and home visits, discussing and updating care/service plans, and supporting continuity of care. Work with care managers, practitioners, and community providers to accommodate changes, respond to member and provider questions, and help identify needs and make referrals. Support documentation and maintenance of member records in line with state and regulatory requirements while educating members on benefits and available resources.
