Key Responsibilities

  • •Provide skills-based individual and group sessions to caregivers in treatment at Equip.
  • •Implement motivational strategies with caregivers to engage with their care plans, including monitoring adherence to care plans and completion of program elements.
  • •Participate in multidisciplinary treatment team huddles to support patient progress in treatment.
  • •Actively listen and guide caregivers to anticipate challenges and engage in coping-ahead strategies.
  • •Maintain an availability calendar allowing caregivers to schedule appointments.

Key Requirements

  • •2+ years of experience working with families or supports in a health care setting (coaching, case management, mentorship, social work, or milieu therapy)

  • •Ability to identify and help patients and families achieve treatment goals
