Care Coordinator II

Coordinate member care by providing outreach via phone or home visits, discussing and updating care/service plans, and facilitating continuity of care. Partner with healthcare and community providers to accommodate progress or changes, support member and provider inquiries, and document member records in line with state and regulatory requirements. Identify needs, make referrals to Care Manager, community organizations, and Disease Manager, and provide education on available benefits and resources.
