Key Responsibilities

  • •Coordinate care activities with care plans, working with healthcare and community providers, partners, members, and caregivers to accommodate progress or changes.
  • •Support member and provider inquiries, requests, and concerns related to the care/service plan.
  • •Communicate with care managers, practitioners, and others to facilitate member services and continuity of care/service.
  • •Maintain member records per state and regulatory requirements, support service assessments/screenings, and make referrals to Care Manager, community-based organizations, and Disease Manager.

Key Requirements

  • •High school diploma or GED.
  • •1–2 years of related experience.
  • •Experience coordinating care for developmentally or intellectually disabled clients or behavioral health clients.
  • •For Arkansas Total Care plan: hold a bachelor’s degree in a social science/health-related field or meet the high-school + experience requirement.
  • •Must reside in Arkansas (or a border city) and complete required travel (30%).
