Job Summary

Support patients with severe mental illness through one-on-one coaching, group activities, and navigation of community resources. Based in our Brooklyn clinic, you’ll work with a multidisciplinary team to provide compassionate, coordinated care, conduct field visits, and help patients achieve personal goals while practicing trauma-informed, recovery-oriented care.

Job Function: Customer Support (Service Desk)

Key Responsibilities

•Provide one-on-one coaching to support members' personal goals, inspire self-discovery, and strengthen skills for care
•Plan and lead group activities that encourage participation, build community, and celebrate progress
•Help members access and navigate community resources and support services
•Conduct field work in the community, including home check-ins and attending events with members as a supportive peer
•Collaborate with teammates and clinical staff to ensure every member receives compassionate, coordinated support

Key Requirements

•NYS Certified Peer Specialist (NYCPS) certification required
•Minimum 2 years of stable, personal recovery
•High school diploma or equivalent required

•Strong coaching and motivational skills, with ability to support individuals through challenges and model effective coping strategies
