Job Summary

Key Responsibilities

  • •Identify, outreach to, engage, and enroll eligible individuals into the program virtually and in-person at partner locations.
  • •Listen to members and meet them where they are, using communication approaches suited to multiple outreaches.
  • •Educate clinic and community partners on Pair Team services and how they link to Enhanced Care Management.
  • •Explain Enhanced Care Management services and benefits while addressing initial hesitancy or distrust.
  • •Manage the end-to-end enrollment funnel, provide updates on blockers/delays, and meet key enrollment metrics.

Key Requirements

  • •You are physically located in the United States and can work PST hours, including required weekend work.
  • •1+ year of experience in a metric or quota-driven environment.
  • •Bilingual in English/Spanish with strong reading, writing, speaking, and listening skills.
  • •You are dedicated to working within and improving the community you are part of, with a proven track record of exceeding performance indicators.
  • •Comfort using multiple technology platforms; you also have a valid driver’s license and auto liability insurance.
