Key Responsibilities

  • •Serve as the first point of contact for inbound patient communication, assess urgency, resolve requests, or triage to the right team member.
  • •Schedule visits with the clinical team, coordinate logistics, and maintain and update patient data accurately across systems.
  • •Manage an active caseload by building individualized care plans, reaching patients via SMS and phone, and researching resources to address barriers.
  • •Follow cases through to confirmed resolution, escalate complex cases for specialist support, and facilitate step-down back to the caseload when appropriate.
  • •Document activities and decisions in the EMR and collaborate with providers through shared patient records to ensure consistent, high-quality care.

Key Requirements

  • •2+ years in care navigation, case management, community health, or patient-facing customer support in healthcare, with virtual and/or phone experience.
  • •Experience with SDOH navigation (housing, transportation, food access, behavioral health, benefits enrollment) or strong patient experience delivery in a high-volume multi-channel environment.
  • •Working knowledge of, or ability to quickly learn, community resources and eligibility rules across Pennsylvania, New Jersey, New York, and/or Delaware.
  • •Proficiency with EMR, ticketing systems, or care management platforms and comfort using digital tools daily.
  • •Strong written communication and ability to independently manage a caseload with careful documentation and accurate patient records.
