Home Based Primary Care Physician (MD/DO)

Provide relationship-centered primary care for Medicare Advantage members with complex medical, functional, and social needs in their homes across New Jersey. Own outcomes for a defined patient panel through comprehensive in-home assessments, proactive identification of clinical changes, and post-discharge transitional visits. Coordinate care across an interdisciplinary team using Clover Assistant and other clinical technology, while documenting thoughtfully to close care gaps and reduce avoidable readmissions.
