•Provide skills-based individual and group sessions to caregivers in treatment at Equip.
•Implement motivational strategies with caregivers to engage with their care plans, including monitoring adherence to care plans and completion of program elements.
•Participate in multidisciplinary treatment team huddles to support patient progress in treatment.
•Maintain an availability calendar allowing caregivers to schedule appointments.
•Conduct proactive outreach (phone calls/messaging), missed appointment recovery, reminders, tracking, and support to ensure full engagement with the care plan.

Key Requirements

•2+ years of experience working with families in a health care setting including coaching, case management, mentorship, social work, or milieu therapy
•Lived experience as an eating disorder support for a loved one in firm/stable recovery
•Ability to connect with a range of diverse people with varying lived experiences
•Ability to identify and direct patients and families in achievement of treatment goals
•Strong oral and written communication skills with the ability to build effective, professional, and collaborative relationships both internally and externally
