Community Resource Coordinator II

Support community connection activities by connecting members to community and care resources and providing health education to assist their care management journey. Coordinate community outreach, respond to member inquiries, and maintain documentation of community resources aligned with health plan requirements. Assist nurses and social workers through outreach and informal guidance, perform non-clinical and non-medical assessments (including home safety and transportation/employment needs), and conduct telephonic or in-person outreach; may make visits as needed.
