Bilingual Case Management Specialist (Remote Flexible, Spanish Speaking)

Provide evidence-based case management for individuals with serious mental illness/substance use disorder and/or homelessness and high medical needs. Conduct ongoing caseload management, relationship-based social support navigation, and periodic telephonic/SMS outreach to ensure timely follow-up. Work with members to set Health Action Plan/Shared Care Plan goals and coordinate appointments and referrals with internal/external partners. Support nurse and behavioral health leaders with delegated tasks and help meet encounter metrics.
