Care Navigator - Foster Care

Develop and coordinate foster care–focused care management activities for members by assessing needs and barriers, creating personalized care/service plans, and educating members and caregivers on benefits and available services. Advocate and coordinate access to providers, specialists, and community resources, monitor progress, and document care management activities for compliance with state and federal payer regulations. Fully remote in Missouri, with possible on-site visits when needed.
