Field Hospital Enrollment Specialist (Bilingual Spanish)

Serve as Pair Team’s on-site presence in hospital emergency departments and inpatient units by identifying eligible Medi-Cal and Medicare patients, educating patients and families about Enhanced Care Management, and enrolling consenting individuals before or shortly after discharge. Partner closely with hospital social workers, case managers, nurses, and registration teams, verify eligibility, document outreach and enrollment activity in the electronic medical record, and coordinate warm handoffs for follow-up.
