Key Responsibilities

  • •Conduct in-home and telehealth visits for Medicare Advantage members.
  • •Perform comprehensive assessments, medication reviews, and chronic disease management.
  • •Develop individualized care plans that address clinical needs and social determinants of health.
  • •Identify barriers to care (fall risks, food insecurity, caregiver challenges, and gaps in treatment adherence) and collaborate with the interdisciplinary team.
  • •Monitor health status and intervene early to help prevent avoidable emergency department visits and hospitalizations.

Key Requirements

  • •Active Nurse Practitioner (NP) license in New Jersey or ability to obtain licensure.
