Key Responsibilities

  • •Provide oversight, mentorship, and support to Medical Directors, Attending Physicians, and Advanced Practice Providers across the region.
  • •Establish and maintain clinical standards to promote quality, compliance, and exceptional patient outcomes.
  • •Lead quality and performance improvement initiatives, including reducing hospital readmissions and improving transitions of care.
  • •Partner with Regional Directors of Operations and Vice Presidents of Operations to drive regional clinical and operational performance.
  • •Collaborate with hospitals, health systems, and referring providers to strengthen post-acute care partnerships and support regional growth initiatives.

Key Requirements

  • •MD or DO from an accredited medical school.
  • •Board Certification in Internal Medicine, Family Medicine, or Geriatrics.
  • •Active and unrestricted medical license in applicable state(s) and active DEA registration.
  • •Minimum of five (5) years of physician leadership experience in post-acute care, senior living, long-term care, or related healthcare environments.
  • •Experience leading value-based care, population health, HCC/risk adjustment, or readmission reduction initiatives (preferred).
