Key Responsibilities

  • •Allocate daily workload within the specialist area, prioritizing caseload demands and managing changing workload needs.
  • •Conduct comprehensive person-centered psychosocial assessments and develop and implement individual care strategies in conjunction with the service user, caregivers, and multidisciplinary team.
  • •Coordinate social care plan delivery, liaise with government or charity agencies, and maintain professional working relationships.
  • •Act as an advocate for patients and ensure expressed wishes are central to planning, including complex discharge planning and smooth transitions of care.
