Key Responsibilities

  • •Support care management activities and teams assigned to members to ensure services are delivered and continuity of care/member satisfaction.
  • •Coordinate care activities based on the care plan/service plan and work with healthcare/community providers, partners, and members/caregivers to accommodate changes or progress.
  • •Provide support for member and provider inquiries, requests, or concerns related to the care plan/service plan.
  • •Perform member outreach via phone or home visits, discuss next steps and resources, and provide ongoing education as appropriate.
  • •Document and maintain member records in accordance with state and regulatory requirements and help with service assessments/screenings when required.

Key Requirements

  • •High school diploma or GED.
  • •1–2 years of related experience.
  • •Reside in Arkansas (or a border city) and be able to support members throughout assigned Arkansas counties (Hempstead and Howard).
  • •Designated as safety sensitive in Arkansas and requires a valid driver’s license, child and adult maltreatment check (before hire and recurring), and a drug screen (at time of hire and recurring).
  • •Ability to identify needs and make referrals to Care Manager, community-based organizations, and Disease Manager, supporting education on benefits and available resources.
