Community Resource Coordinator II

Support community connection activities by connecting members to community and care resources to aid their care management journey. Coordinate community outreach, handle member inquiries, document and maintain community resources in line with health plan requirements, and assist the clinical team of nurses and social workers. Perform non-clinical and non-medical assessments (including home safety and transportation/employment needs), conduct telephonic/in-person outreach, and may make home or community organization visits.
