Key Responsibilities

  • •Assist with developing, assessing, and coordinating holistic care management activities and personalized service care plans for long-term care members.
  • •Evaluate member needs and available resources, recommend and/or facilitate the plan for the best outcomes, and identify providers and community resources needed.
  • •Coordinate between members (and/or family/caregivers) and the care provider team to ensure services are accessible.
  • •Provide resource support and education to members and families/caregivers on procedures, service options, referrals, and healthcare benefits.
  • •Collect, document, and maintain LTSS member information and care management activities to ensure compliance; monitor care plans and outcomes and provide recommendations to leadership.

  • •Bachelor's degree and 1 year of related experience.
  • •For Illinois Plan only: must be RN licensed in Illinois, or have a bachelor’s/master’s degree in a human services-related field (as defined in the posting), or be an LPN with 1 year of experience conducting comprehensive assessments and providing formal service for the elderly.
