Care Coordinator II

Support care management for assigned members to help ensure services are delivered by healthcare providers and partners and that care continuity and member satisfaction are maintained. You will conduct member outreach via phone or home visits, coordinate care plan activities with providers and community partners, document assessments and member records, handle member/provider inquiries, and make referrals to Care, community, and Disease Management as needed.
