Key Responsibilities

  • •Conduct in-home and telehealth visits for Medicare Advantage members
  • •Perform comprehensive assessments, medication reviews, and chronic disease management
  • •Develop and implement individualized care plans addressing clinical and social determinants of health
  • •Identify barriers to care, including fall risks, food insecurity, caregiver challenges, and gaps in adherence
  • •Collaborate with physicians, care coordinators, social workers, and other interdisciplinary team members to deliver holistic care

Key Requirements
