Job Summary

Key Requirements

  • •1+ years of professional experience in case management, care coordination, or community-based services.
  • •Be physically located in or near Orange County with familiarity of the local community and resources.

  • •Hold a valid driver’s license and current auto liability insurance.
  • •Strong empathy, cultural fluency, and ability to build trust with diverse populations while documenting work and learning new systems.
