Key Responsibilities

  • •Develops, assesses, and facilitates complex care management activities and personalized care plans for members.
  • •Evaluates member needs and barriers to care, including social determinants of health, and recommends/facilitates best-outcome plans.
  • •Develops ongoing care/service plans and collaborates with providers to identify specialists and community resources.
  • •Coordinates between members (and caregivers) and care teams to ensure person-centered care and adequate service delivery.
  • •Performs outreach, monitors member status and progress, documents care management activities for compliance, and provides education on disease processes and care options.

Key Requirements
