Key Responsibilities

  • •Conduct in-home assessments for Aging and Physical Disabilities Waiver members.
  • •Develop and coordinate holistic service care plans/service plans and recommend best outcomes.
  • •Coordinate access to providers, specialists, and community resources for members and caregivers.
  • •Provide resource support across needs such as employment, housing, independent living, justice, and foster care based on assessments and plans.

Key Requirements

  • •Bachelor's degree and 1 year of related experience.
  • •For Illinois Plan Only (Physically Disabled/Elderly): RN licensed in Illinois OR a human services-related Bachelor’s/Master’s degree OR LPN with 1 year conducting comprehensive assessments and providing formal service for the elderly.
  • •Reside within 30 minutes of the Elgin, IL area.
  • •Past case management, advocacy, or home visits/community travel experience.
  • •Conduct in-home assessments and develop/coordinated service care plans for long-term care members.
