CA Licensed Bilingual Spanish/English Clinician - LCSW or LPCC - Behavioral Health

Deliver evidence-based behavioral health interventions using the Collaborative Care Model for pediatric and/or adult patients with mild to moderate symptoms. You’ll screen and monitor patients, support care coordination with primary care and other providers, track outcomes via a registry, and document treatment recommendations in Salesforce and the electronic health record. The role also includes psychotropic adherence support, relapse prevention planning, patient education, and regular supervision/caseload consultations.
