Key Responsibilities

  • •Provide care to patients based on the needs of an assigned Care Pod and customize treatment plans
  • •Coordinate behavioral health care with primary care providers and other service providers when appropriate
  • •Screen patients for behavioral health and substance use disorders using tools such as PHQ9, GAD7, and Columbia Suicide Risk Assessment
  • •Monitor symptoms and treatment side effects/complications to support symptom reduction
