Key Responsibilities

  • •Review caseload, prepare technology, and navigate your daily schedule to support patient care.
  • •Conduct virtual adult and/or pediatric wellness visits, health assessments, and proactive health risk assessments.
  • •Perform medication reviews and cognitive screenings, providing patients educational information about health and well-being.
  • •Collaborate with Sprinters to evaluate vital signs, arrange blood draws, and address patient-specific needs.
  • •Provide care coordination and management, including preventive interventions, medication management, referrals, community resources, and documentation of findings.

Key Requirements

  • •Board Certified as a Family Nurse Practitioner.
  • •Active Family Nurse Practitioner License in California.
  • •Credentialed Medicaid provider for the state of California (Medi-Cal).
  • •Technologically savvy and comfortable using laptops/mobile devices for charting and HIPAA secure messaging apps for care coordination.
  • •Strong written and verbal communication skills and ability to work independently or in a team environment.
