Job Summary

Deliver virtual adult and/or pediatric wellness care to patients in their homes. Conduct comprehensive wellness exams including health risk assessments, medication reviews, and cognitive screenings, and provide patient education. Coordinate with Sprinters for vital sign evaluation and arranging blood draws, and manage referrals, community resources, and documentation. Review your caseload daily, prepare technology and tools, and work independently while maintaining high-quality, compassionate care.

Key Responsibilities

•Review caseload, prepare technology, and navigate your daily schedule to support patient care.
•Conduct virtual adult and/or pediatric wellness visits, health assessments, and proactive health risk assessments.
•Perform medication reviews and cognitive screenings, providing patients educational information about health and well-being.
•Collaborate with Sprinters to evaluate vital signs, arrange blood draws, and address patient-specific needs.
•Provide care coordination and management, including preventive interventions, medication management, referrals, community resources, and documentation of findings.

Key Requirements

•Board Certified as a Family Nurse Practitioner.
•Active Family Nurse Practitioner License in California.
•Credentialed Medicaid provider for the state of California (Medi-Cal).
•Technologically savvy and comfortable using laptops/mobile devices for charting and HIPAA secure messaging apps for care coordination.
•Strong written and verbal communication skills and ability to work independently or in a team environment.
