Care Navigator - Foster Care

Develop and coordinate care management for foster care members by assessing needs, barriers, and available resources to create and maintain personalized care plans. Provide psychosocial and resource support, coordinate between members/caregivers and care teams, and help ensure timely access to services. Monitor member progress, document care management activities for compliance, and may conduct on-site visits while educating members and caregivers on care options and benefits.
