Key Responsibilities

  • •Support care management activities and assigned teams to ensure services are delivered by providers and partners.
  • •Conduct member outreach telephonically or through home visits; document care plans/services and discuss next steps, resources, questions, or concerns.
  • •Coordinate care activities based on the care plan/service plan and work with healthcare/community providers and members/caregivers to accommodate changes.
  • •Respond to member and provider inquiries related to care plans/services and communicate with care managers and practitioners to ensure continuity.
  • •Maintain member records in accordance with state and regulatory requirements and support referrals and education on benefits/resources.

Key Requirements

  • •High school diploma or GED.
  • •1–2 years of related experience.
  • •For Arkansas Total Care plan: Bachelor’s degree in a social science/health-related field, or high school diploma with at least 1 year coordinating care for developmentally/intellectually disabled clients or behavioral health clients.
  • •Must reside in Arkansas (AR) or a border city.
  • •Must have a valid driver’s license and be able to complete required safety-sensitive checks (child and adult maltreatment check) and drug screen.
