Key Responsibilities

  • •Review your case load and prepare technology/tools to service patients.
  • •Navigate your schedule and partner with in-home Sprinters visiting each patient’s home.
  • •Build meaningful connections with a diverse range of patients while collecting relevant information and performing services.
  • •Conduct proactive wellness care including health risk assessments, medication reviews, cognitive screenings, and patient education.
  • •Coordinate comprehensive care, including preventive interventions, medication management, referrals, community resources, and documentation of findings.

Key Requirements

  • •Board Certified as a Family Nurse Practitioner.
  • •Active Family Nurse Practitioner License in Michigan.
  • •Consistently demonstrate professionalism, integrity, accountability, confidentiality, care, and compassion.
  • •Technologically savvy—comfortable using laptops or mobile devices for charting and HIPAA secure messaging apps for care coordination.
  • •Pass national background check and complete valid clinical license search.
