Key Responsibilities

  • •Support care management activities and the teams assigned to members to ensure services are delivered and continuity of care and satisfaction are achieved.
  • •Perform member outreach via phone or home visits, discuss care/service plan next steps and resources, and provide ongoing education as appropriate.
  • •Coordinate care activities based on the care/service plan and work with healthcare and community providers, partners, and members/caregivers to accommodate changes.
  • •Support member and provider inquiries and requests related to the care/service plan, communicating with care managers and practitioners as needed.
  • •Document and maintain member records according to state and regulatory requirements; may support service assessments/screenings and identify needs for referrals to Care Manager and community-based organizations.

Key Requirements

  • •High school diploma or GED, plus 1–2 years of related experience.
  • •For the Arkansas Total Care plan: a bachelor’s degree in a social science/health-related field, or a high school diploma with at least 1 year coordinating care for developmentally/intellectually disabled clients or behavioral health clients.
  • •Designated as safety sensitive in Arkansas: must have a driver’s license and complete child and adult maltreatment checks (before hire and recurring) and a drug screen (at time of hire and recurring).
  • •Must reside in AR (or border city).
  • •For field support: must reside in or be able to support members throughout Washington and Benton counties in Arkansas.
